Medications at School

If it is necessary for your child to take medications at school, the district wide policy must be followed. If you have any questions regarding this policy, please contact the school nurse.


  1. All medications, prescription or over-the-counter, requires a Physician Authorization Form which may be downloaded from this page. The form must be completed entirely, including the signature from the physician and parent.


  2. All medications, prescription and over-the-counter, must be stored in the school office. The medication must arrive in the original container with student's name.


  3. A new form with updated information must accompany any changes in the dose, time, or medication.


  4. Any medication not picked up at the close of the school year will be discarded.


  5. Your child will be subject to discipline if the above policies are violated.
